Advanced Primary Care - Health home implementation project through UHS

ReACH is developing a comprehensive, longitudinal database of patient health care services delivered by UHS and UT Medicine providers. The Center will augment these administrative data with primary data collected from patients and providers to create a unique research resource for studies of quality of care, health disparities, and innovative models of care. Center will also partner with other South Texas health care providers to evaluate effective models of care for their populations. Because San Antonio boasts one of the largest military complexes in the nation, a research partnership with the military will be a key focus of advancing the health of all residents of the Bexar County community.
